Irritable Bowel Syndrome (IBS) – What is it?
IBS or Irritable Bowel Syndrome is a common bowel disorder consisting of a set of bowel symptoms characterised by bowel pain, bloating and change in stool consistency and frequency. Constipation and diarrhoea are usually associated to irritable bowel syndrome.

The symptoms of irritable bowel syndrome may vary widely over time and individuals, but abdominal pain especially in the region of the colon, is always present and is associated with distrubed bowel movements.
Irritable Bowel Syndrome is a “functional gastrointestinal disorder” (FGID) because it is a disorder of the movement and sensation of the colon. It can easily go undetected as symptoms range from mild to severe in nature and affected individuals usually treat these bowel symptoms in isolation under their daily activities and work are affected. Although irritable bowel syndrome does not lead to obstruction, bleeding or cancer of the colon, individuals with severe symptoms may experience impaired quality of life with major disability.
Diagnosis of Irritable Bowel Syndrome
The diagnosis of irritable bowel syndrome is made based on chronic recurrent bowel symptoms, in an otherwise healthy individual with no alarm features to suggest other colonic diseases. The important alarm features are:
- unintentional weight loss
- nocturnal attacks
- bloody stool or bleeding from rectum
- symptoms of anaemia eg pallor, breathlessness, lethargy, giddiness
- fever with unknown cause
- change in usual bowel symptoms
- onset in older patients (>35 years old)
- family history of colonic cancer/inflammatory bowel disease

Any individual with alarm features mentioned above should seek medical help immediately so that blood and stool tests can be done to exclude systemic or colonic diseases that can mimic irritable bowel syndrome. Depending on the clinical presentation, your doctor may also advise you to go for a barium enema x-ray in which a chalky substabce (Barium) is administered through the rectum into the colon so that x-ray pictures of the colon can be obtained.
Alternatively, colonscopy involving the use of a flexible tube mounted with a camera may be performed to check for abnormality in the colon. In people with irritable bowel syndrome, all these tests will be normal ad a confident diagnosis can be made.
